COUNTY_NAME
String
The county where the facility is located, obtained from the facility's license application form. Based on COUNTY_CODE from the FACILITY table in the state Electronic Licensing Management System (ELMS). See Lookup table.
PROVIDER_TYPE
String
The provider type. Values include: Long-Term Care, Non-Long-Term Care, All.
FAC_FDR
String
The facility type group that CHCQ uses to crosswalk between differences in license and certified facility types. See Lookup table.
COUNT
Numeric
The number of facilities for a given set of: STATE_FISCAL_YEAR-COUNTY_NAME-PROVIDER_TYPE-FAC_FDR.
ANNUAL CHANGE (DIFF)
Numeric
The difference in the number of facilities in a given year, compared to the previous year.
